We are seeking an experienced Home Care Physical Therapist Assistant (PTA) to join our team at Liberty Health. In this role, you will play a vital part in providing patient care services along with direct treatment and follow-up as assigned by the Physical Therapist.

The ideal candidate will have completion of an accredited physical therapy assistant program and current State licensure. A minimum of one year's therapeutic experience in a non-home health care environment is preferred, with strong knowledge of theory and concepts relating to the practice of physical therapy.

You will be responsible for updating the care plan in coordination with the therapist and physician, and planning monthly patient re-evaluations with the therapist. You will also document patient care data accurately, thoroughly, and in a timely manner, reflecting evidence of improvement/progress/decline in patient's condition.

As a Home Care Physical Therapist Assistant, you can expect a competitive salary range of $45,000 - $60,000 per annum, depending on experience. We also offer a comprehensive benefits package, including paid time off, health insurance, and retirement savings plan.
