A highly skilled and compassionate Clinical Care Coordinator is needed to join the Veterans Affairs, Veterans Health Administration team. The VA Community Care Coordination Program is seeking a dedicated and organized professional to oversee the coordination and oversight of healthcare provided to Veterans authorized to receive care in a non-VA setting. As a Registered Nurse, you will be responsible for directing staff and supporting program components. With a strong focus on patient-centered care, you will ensure that services are provided in the appropriate setting and level of care, maximizing efficient and cost-effective use of health care resources.

The Non-VA Care Nurse Coordinator will utilize all available resources to provide safe, efficient, and timely coordination of quality healthcare services. You will serve as a liaison between Veterans and medical facility staff, assisting with scheduling appointments, transferring records, and arranging provider-to-provider contact. Additionally, you will facilitate care needs for Veterans requiring inpatient, outpatient, or home health services. With a passion for delivering exceptional patient care, you will assume responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, and patient self-management after discharge.

This role offers a competitive salary of $ $94,459 - $124,037 per year, depending on experience.
