Primary Responsibilities:

  • Develop and implement comprehensive care plans to meet patient needs
  • Facilitate collaboration among healthcare teams to ensure smooth patient transitions
  • Maintain accurate and up-to-date patient records and databases

Requirements:

  • Associate's degree in a related field
  • Minimum two years of experience in a healthcare setting, preferably in a leadership or supervisory capacity

Compensation Package: Estimated salary range $19.98 - $26.04/hour
