Job Summary

The Palliative Care Nurse Practitioner will work collaboratively with referring providers and consultants to provide relief of suffering for patients and families facing serious illness. Key responsibilities include:

  • Assessing and managing symptoms such as pain, dyspnea, anxiety, and fatigue
  • Developing and implementing individualized care plans to meet the unique needs of each patient
  • Collaborating with interdisciplinary teams to ensure comprehensive and coordinated care
  • Providing education and support to patients and families to promote self-management and empowerment
  • Participating in quality improvement initiatives to enhance patient outcomes and experiences
Requirements

To be successful in this role, the ideal candidate will possess:

  • A master's degree in nursing and certification as a Family/Geriatric or Adult Nurse Practitioner
  • Minimum of one to three years' experience as a nurse practitioner or physician's assistant, preferably in palliative care
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
  • Knowledge of pain and symptom management, as well as experience with hospice and palliative care
  • Current licensure and certification in the state of Rhode Island and Massachusetts
