We are seeking a highly skilled Palliative Care Advanced Practice Provider to join our team at Genesis HealthCare, Inc. This is a full-time opportunity to provide primary and urgent care services to nursing center residents in collaboration with the primary care team.

The successful candidate will work closely with the Center Leadership Team to achieve center-specific goals and provide patient-specific medical care. Emphasis will be on the care of the individual patient and family with a focus on pain and symptom management, psychosocial support, and advance care planning.

The Palliative Care Advanced Practice Provider will serve as a specialized consultant within the interprofessional team, providing recommendations for the plan of care while incorporating patient goals and preferences.

Key qualifications include a graduate degree from an accredited School of Nursing program, licensure as a registered nurse, and certification as a Nurse Practitioner or Physician Assistant. A minimum of two years' experience working with the adult or geriatric population is preferred.
