Clinical Coordinator RN

Tufts Medicine Care at Home is seeking a skilled Clinical Coordinator RN to join our team. As a Clinical Coordinator RN, you will play a vital role in the coordination of patient care, working closely with our clinical staff to ensure the delivery of high-quality services to patients and families in their homes.
Key Responsibilities:- Triage calls related to patient care, including physician orders, staff issues, and other clinical issues.
- Work closely with clinical staff to provide necessary services for patients and families in a timely manner, including direct patient care.
- Communicate regularly with Clinical Managers and the Clinical Director on clinical practice and performance-related issues.
- Demonstrate expertise and knowledge in areas such as current methods of care and treatment, policies, procedures, and protocols specific to the Agency, and the Agency Infection Control Plan.
- Assist with orientation and preceptorship of clinical staff.
- Evaluate performance of each individual in relation to established standards in collaboration with the clinical manager.
- Determine staff competency in a fair and consistent manner, working collaboratively with all disciplines to facilitate productivity, cooperation, and efficient operations.
- Triages calls from physicians and follows through on required actions, including communication to field staff, changes in orders, scheduling PRN visits, and follow-up as a clinical resource to clinical staff if needed.
- Provide documentation that is concise, accurate, and informative, and which supports reimbursement.
- Practice in accordance with standards of care, policies, procedures, and scope of practice, demonstrating an awareness of regulatory guidelines.
- Ensure compliance within guidelines set forth by regulatory agencies (DPH, ERISA, etc.) and demonstrate compliance with Tufts Medicine Care at Home policies and procedures.
Requirements:
- Three years of community health experience required or a combination of at least one year of acute medical/surgical nursing experience and two years of home health experience required.
