Work Schedule: Full Time

Shift: Days, Monday - Friday 9am to 4:30pm, including Weekends and Holidays

Your Role:

The RN Care Manager collaborates with patients, families, social workers, nurses, physicians, and the interdisciplinary team to ensure coordinated, patient-centered care throughout the healthcare continuum. This role is vital for managing efficient and cost-effective care by monitoring resources and escalating clinical care as necessary. Under the guidance of the Care Management Supervisor or Manager, the RN Care Manager is responsible for evaluating post-hospital needs, developing transition of care plans, and initiating their implementation prior to patient discharge. Key responsibilities include optimizing patient flow, enhancing continuity of care, ensuring patient satisfaction and safety, preventing readmissions, and managing length of stay. Daily communication with the interdisciplinary team during rounds is essential. Core competencies include care coordination, discharge planning, and understanding medical necessity.

· Conduct initial evaluations for transition of care needs within one calendar day of admission, documenting in accordance with policies.

· Review essential patient information, including labs, medications, and medical history.

· Integrate patient and family care goals into transition planning, ensuring effective communication with the multidisciplinary team.

· Address clinical, social, and financial factors in the transition of care plan.

· Meet with patients and families to discuss realistic discharge options and post-hospital care providers.

Qualifications:

Essential Expertise:

· Associate's Degree and a minimum of 2 years of experience

· Active Registered Nurse License
