Position Summary
This role involves delivering direct patient care to Veterans in a dynamic outpatient environment. Responsibilities include managing scheduled appointments and addressing walk-in patients' needs. Our approach is centered on patient care and teamwork, focusing on creating tailored care plans that emphasize preventive health and education.
Key Responsibilities
- Conduct video consultations with patients.
- Facilitate patient and family education, particularly in chronic disease management.
- Collaborate with the Patient Aligned Care Team (PACT) for effective patient care coordination.
- Provide coaching on healthy lifestyle choices.
- Perform patient assessments and apply critical thinking skills.
- Utilize procedural skills and manage computerized health records.
- Deliver excellent customer service to all patients.
