Job Overview

At Accord, we are seeking a skilled Case Manager to join our team. As a Case Manager, you will play a vital role in enhancing the quality of life for individuals with disabilities. Your primary responsibility will be to assess the needs of our clients, develop personalized care plans, and coordinate services to meet their unique requirements.

Key Responsibilities:

  • Develop and implement community support plans for individuals with disabilities.
  • Assist clients in accessing services, developing service agreements, and documenting case notes.
  • Inform clients and their guardians of service options that align with their needs.
  • Meet with clients regularly to evaluate progress and adjust care plans as needed.
  • Identify and engage potential providers to support clients in achieving their goals.
  • Evaluate and monitor the effectiveness of services identified in care plans.
  • Ensure compliance with case management rule 185 and relevant state regulations.

Requirements:

  • Bachelor's degree in Human Services, Social Work, Psychology, Sociology, or a related field.
  • OR Bachelor's degree in any other field with at least one year of experience working with individuals with mental illness or disability.
  • Excellent written and oral communication skills.
  • Ability to work independently and in crisis situations.
  • Attention to detail and process-oriented.
  • Able to pass a DHS background check and maintain a valid driver's license.

Work Environment:

The Case Manager will work primarily in the community and office setting, with some flexibility based on program needs. The position requires a full-time, non-exempt schedule of 40 hours per week.
