Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Mitchell Martin in Brooklyn, NY.

Job Summary:

The Licensed Practical Nurse will assist the Registered Nurse in implementing the nursing process on assigned patients. The LPN will work as a member of the overall nursing team to promote, provide, and maintain high-quality care to patients, in accordance with the New York State Nurse Practice Act.

Responsibilities:
  • Work with members of the medical team to provide personalized high-quality healthcare.
  • Ensure patients are prepared to be seen by physicians.
  • Plan nursing care for patients.
  • Assist physicians and mid-level providers with procedures.
  • Administer medication and/or treatment when indicated.
  • Administer medication therapy and vaccines as ordered by the provider.
  • Maintain and secure all clinical supplies and medication in the service area.
  • Makes follow-up and referral appointments.
  • Conducts patient teaching and counseling; provide patients with health information materials.
  • Makes internal and external referrals for specialties and follows up with their result.
  • Maintains proper documentation in patient's electronic medical record.
  • Performs and/or assists patients with treatments/procedures.
  • Ensures PCAs are conducting pre-visit planning for all patients daily.
  • Observes, reports, and documents significant physical, psychological, or safety status changes in patient's condition to the RN/Provider promptly.
  • Communicates patient care needs to ancillary staff and follow up accordingly.
  • Performs routine screening tests as needed, specific to each unit.
  • Maintains an orderly and safe environment and adheres to infection control guidelines.
  • Manages client information following HIPAA guidelines.
  • Conducts an audit of all Oral/Injectable medications.
Requirements:
  • Graduate of an accredited school of Practical Nursing.
  • Currently holds New York State License, Certification, and Registration.
  • One year of previous related nursing experience preferred.
  • BCLS, Infection Control, and Child Abuse certificates required.
  • Bilingual in Spanish or Creole is a plus.
