Job Title: Licensed Practical Nurse

Services for the UnderServed, Inc. is seeking a skilled Licensed Practical Nurse to join our team at our men's shelter in Brooklyn. This is a unique opportunity to make a difference in the lives of our clients and contribute to the delivery of high-quality healthcare services.

Job Summary:

The Licensed Practical Nurse will work closely with our Program Director, Director of Social Services, Registered Nurse, and Social Service staff to coordinate patient care and provide medical support to our clients. This is a challenging and rewarding role that requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Provide medical care and support to clients, including administering medications, taking vital signs, and conducting basic medical procedures.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Communicate effectively with clients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance patient care and services.
Requirements:
  • Current licensure as a Licensed Practical Nurse in the state of New York.
  • Minimum of 1 year of experience in a healthcare setting, preferably in a social services or community-based organization.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
