Job Title: Licensed Practical Nurse

Job Summary:

We are seeking a highly skilled and compassionate Licensed Practical Nurse to join our team. As a Licensed Practical Nurse, you will work under the direction of a registered nurse, physician, or other licensed healthcare provider to provide nursing care tasks and procedures consistent with individual treatment plans and scope of practice.

Key Responsibilities:
  • Administer medications and nursing procedures to individuals as prescribed.
  • Obtain and record individuals' temperatures, blood pressures, pulse rates, and other vital signs using standard instruments and procedures.
  • Monitor and record individuals' symptoms and conditions to obtain and provide information for diagnosis and treatment.
  • Assist registered nurses, physicians, or other licensed healthcare providers in the identification of individuals' needs and participate in the development of plans to address goals.
  • Prepare individuals for or assist with examinations and treatments.
  • Maintain charts and other records of individuals' conditions and/or administered treatments.
  • Attend to individuals' personal needs, including activities of daily living and hygiene tasks, such as bathing, dressing, feeding, and grooming.
  • Communicate with individuals, listen to their concerns and responses, and encourage them to cooperate and participate in treatment and rehabilitation activities.
  • Provide education and counseling to individuals and/or families regarding their health, medications, treatment goals and procedures, and behavioral interactions as appropriate.
Requirements:
  • A license and current registration to practice as a practical nurse in New York State.
  • OR a limited permit to practice as a practical nurse in New York State.
  • OR an application on file for a limited permit to practice as a practical nurse in New York State.
Work Environment:

This position covers two locations: Canarsie SOIRA and 64th St SOIRA. The work schedule is 2:00pm-10:30pm, with pass days on Thursday/Monday (week 1) and Saturday/Sunday (week 2).
