Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Centers Plan for Healthy Living. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

The Licensed Practical Nurse will work under the direction of a Registered Nurse to provide skilled nursing care to clients in their homes. This includes administering medications, performing treatments, and documenting patient progress.

Responsibilities:
  • Provide skilled nursing care to clients in their homes, including medication administration and treatment as directed by the Registered Nurse and Physician's Plan of Care.
  • Assess client physical and psychosocial status changes and report findings to the Registered Nurse.
  • Document and report all changes in patient status, accidents, incidents, events, and injuries.
  • Follow agency protocols, policies, and procedures regarding infection control and disposal of biomedical waste.
  • Attend case conferences, meet with quality improvement coordinators, and communicate regularly with the COC as requested.
  • Provide for client safety and respond to any signs of neglect or abuse by alerting the COC or PSM immediately.
  • Regularly update personnel records and respond to requests made by the HR department.
  • Perform other duties as assigned by a COC, PSC, or PSM.
Requirements:
  • Active NYS LPN license.
  • Must have NYS Driver license and access to a vehicle.
