The Role

The Resource Specialist - Housing Transition Expert focuses on assisting veterans and their families in achieving successful housing transitions. This involves creating housing service plans, identifying necessary resources, and facilitating processes to ensure housing stability.

Key Responsibilities
  • Maintain a caseload of veterans with significant barriers to housing, providing individualized support and guidance.
  • Conduct intake and eligibility meetings, completing necessary forms to determine eligibility based on need and program requirements.
  • Perform home visits to assess participants' needs and develop personalized housing service plans.
  • Work closely with Contract Specialists to verify service determinations and communicate with participants.
  • Continuously follow up with participants, conducting ongoing assessments and re-certifications as per program guidelines.
  • Coordinate with other service providers and Benefits Specialists as needed to ensure comprehensive support.
  • Ensure 80% of households served exit with permanent housing, meeting program goals.
  • Complete weekly and monthly reports on participant status, maintaining accurate records.
  • Register participants for the SSVF satisfaction survey, promoting program evaluation and improvement.
  • Participate in training and monitoring activities, including record reviews and webinars.
Additional Responsibilities
  • Maintain accurate and confidential participant files, adhering to grant and confidentiality requirements.
  • Ensure a 95% accuracy rate on data entry and file audits, maintaining high standards of quality.
  • Protect participant confidentiality and maintain statistical data as required by grant funding sources.
  • Adhere to contractual agreements related to resource and case management services.
  • Implement public health and safety guidelines related to COVID-19 in all practices.
  • Attend meetings with partnering agencies and Primavera Foundation staff as scheduled.
Qualifications
  • Knowledge of issues facing veterans and low-income/homeless individuals, including housing, employment, mental, and physical health.
  • Strong interviewing and assessment skills, with the ability to work effectively with program participants, the public, staff, and volunteers.
  • Effective communication and writing skills, with proficiency in office computer systems and software.
  • Ability to function with minimal supervision, accurately tracking information and services in ServicePoint.
Minimum Qualifications
  • Bachelor's degree in Social Work or a related field.
  • A minimum of two years of work experience in social services, providing case management services.
  • One year of experience working with low-income or homeless individuals and/or veterans. Note: A combination of relevant education and professional experience may be considered in lieu of degree.
  • First Aid/CPR certification or ability to obtain.
  • Level 1 fingerprint clearance card or ability to obtain.
  • Reliable transportation with current registration, a valid driver's license, a clean driving record, current registration, and proof of insurance coverage.
Preferred Qualifications
  • Bilingual (English/Spanish).
  • Military veteran or veteran family member.
  • Experience conducting home visits.
  • Experience working in a program providing rent assistance.
  • One year of experience and demonstrated proficiency with web-based database program(s).
