The Community Resource Specialist plays a crucial role in connecting individuals with essential services and support. This position involves conducting satisfaction assessments and engaging in special initiatives to enhance member involvement. The specialist will initiate outbound communications, facilitate three-way conversations, and respond to incoming inquiries to assist members in fulfilling their identified needs by determining the most suitable resources available to them.

Key Responsibilities:

  • Provide essential information and resources to clients through various communication channels including phone, text, chat, and email.
  • Work closely with patients to address social determinants of health (SDoH) challenges, aiding them in managing their overall health effectively.
  • Conduct follow-up communications to verify that referrals have been received and understood by members.
  • Facilitate three-way calls with community-based organizations, health insurance providers, and medical offices.
  • Assist members in accessing vital resources such as food assistance, housing support, and childcare services.
  • Guide members through the application processes for government benefits including Social Security Disability Insurance (SSDI), Supplemental Security Income (SSI), and Medicaid.
  • Support members in completing health plan Health Risk Screenings.
  • Help members schedule necessary medical appointments.
  • Contribute to the development of a comprehensive repository of community resources in the areas served by Pyx Health.
  • Collaborate with agency partners to meet the needs of clients and community members.
  • Deliver accurate information, referrals, and advocacy to all clients.
  • Provide outstanding customer service and identify opportunities for enhancing service delivery efficiency.
  • Engage in weekly follow-ups with select clients to monitor outcomes and explore alternative options.
  • Gather feedback from members regarding their experiences with Pyx Health.
  • Maintain thorough documentation of all member interactions.
  • Record precise and detailed notes in a web-based platform.

Qualifications and Skills:

  • High School Diploma or equivalent; further education, particularly in management, is advantageous.
  • Bachelor's degree or a minimum of 3 years of professional experience in the healthcare sector.
  • Bilingual candidates are preferred.
  • Strong commitment to helping others achieve healthier lifestyles.
  • Excellent critical thinking and problem-solving abilities.
  • Proficient in relevant computer applications and capable of quickly learning new online tools.
  • Ability to work independently in a remote setting.
  • Proficient with technology, particularly computers, software applications, and telecommunication systems.
  • Exceptional verbal and written communication skills.
