Nurse Practitioner/Physician Assistant

Job Summary
We are seeking a skilled Nurse Practitioner/Physician Assistant to join our team in Prior Lake, MN. As a member of our team, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
Responsibilities
- Conduct comprehensive health assessments, including medical history, physical examination, and laboratory results
- Develop and implement individualized care plans to address patient needs and goals
- Collaborate with healthcare providers, case managers, and other stakeholders to ensure seamless care coordination
- Provide education and support to patients and their families to promote health and wellness
- Document all patient interactions, assessments, and care plans in accordance with Matrix guidelines and protocols
Requirements
- Master's degree in Nursing or Physician Assistant Studies
- Current RN and NP or PA licensure in the state of practice
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant
- Strong communication and interpersonal skills
- Ability to travel to patient homes and skilled facilities
