Job OverviewWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes.
Responsibilities- Conduct adult and geriatric assessments, medical history reviews, and provide health education to patients
- Collaborate with physicians, case managers, and other healthcare professionals to ensure comprehensive care
- Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
- Document patient information accurately and thoroughly
- Masters degree in nursing or a related field
- Current RN and NP or PA licensure in the state of practice
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software

Ability to travel to meet patients in their homes, valid state driver's license, and adequate automobile insurance coverage.
