Job SummaryWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in patients' homes, identifying potential health risks and developing personalized care plans to improve health outcomes.
Responsibilities- Conduct home visits to assess patients' health status and identify potential risks
- Develop and implement personalized care plans to address identified health risks
- Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination
- Provide education and support to patients and their families to promote healthy behaviors and self-management
- Document patient information and care plans in a timely and accurate manner
- Master's degree in Nursing or Physician Assistant studies
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of practice
- Board certification in a relevant specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
