Job SummaryWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a PRN Home Risk Assessor. In this role, you will conduct comprehensive health assessments, review medical history, and provide education and support to patients and their families.
Responsibilities- Conduct adult and geriatric assessments, including medical history, diagnosis, and treatment
- Provide health education and support to patients and their families
- Collaborate with physicians, case managers, and other healthcare professionals to ensure comprehensive care
- Document patient information and outcomes according to Matrix guidelines and protocols
- Masters degree in nursing or physician assistant studies
- Current RN and NP/PA licensure in state of practice
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
