Key Responsibilities- Conduct thorough health assessments in patients' homes, including medical history, medication adherence, and social environment
- Identify potential health risks and develop personalized care plans to address these risks
- Collaborate with primary care physicians and other healthcare professionals to ensure seamless care coordination
- Provide education and support to patients and their families to promote healthy behaviors and disease management
- Master's degree in Nursing or Physician Assistant studies
- Current RN and NP/PA licensure in state of practice, with prescription authority
- Board certification in a Matrix-approved specialty
- Minimum 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
