Job SummaryWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team in Sherburne, NY. As a PRN/Part Time provider, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
Responsibilities- Conduct comprehensive health assessments, medical history, diagnosis, and treatment
- Provide health education and counseling to patients and their families
- Collaborate with physicians, case managers, social workers, and other healthcare professionals
- Document patient information accurately and thoroughly
- Participate in quality improvement initiatives and contribute to the development of best practices
- Master's degree in Nursing or Physician Assistant studies
- Current RN and NP or PA licensure in the state of practice
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
