Job Summary

We are seeking a highly skilled Licensed Practical Nurse to join our Primary Care team. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Key Responsibilities
  • Provide comprehensive nursing care to patients in a primary care setting, including assessment, planning, implementation, and evaluation of patient care.
  • Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Administer medications and treatments as prescribed by physicians, and monitor patients for adverse reactions.
  • Conduct patient education and health promotion activities to empower patients to take an active role in their healthcare.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Graduate of an accredited Licensed Practical Nursing program.
  • Current licensure as a Licensed Practical Nurse in the state of practice.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
  • Ability to maintain confidentiality and handle sensitive patient information with discretion.
