Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our Primary Care team at the Veterans Affairs, Veterans Health Administration. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our team.

Key Responsibilities
  • Provide nursing care to adult and geriatric patients in a Primary Care setting, working collaboratively with the Patient Aligned Care Team (PACT) to ensure efficient workflow and comprehensive health care plans.
  • Conduct nursing intakes, evaluate patient conditions, and provide care consistent with licensure, certification, and functional statement.
  • Develop and implement care management plans, collaborating with PACT staff to ensure comprehensive patient care.
  • Manage clinic workflow, ensuring timely placement of patients in examination rooms and providing direction to patients throughout the clinic environment.
  • Participate in team performance improvement and sustainment activities to optimize team efficiency and care delivery.
  • Communicate effectively with patients, family members, and healthcare team members, using informal and formal communication methods to facilitate safe transitions of patients between care settings.
  • Provide education to patients and/or family members on disease processes, medication, and prescribed treatment regimens, collaborating with RNs and healthcare teams as needed.
  • Recognize urgent or emergent patient care situations, seeking assistance from RNs and/or providers, and initiating emergency interventions as directed.
Requirements
  • Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or the Accreditation Commission for Education in Nursing.
  • Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth of the United States, or District of Columbia.
  • Completion of at least 1 year of qualifying experience at the GS-5 level or equivalent, demonstrating the following KSAs: technical proficiency in initiating, performing, and completing assigned duties in providing care to variable patient populations, knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions, and ability to recognize, report, and record relevant patient information.
