Nursing Care Coordinator

We are seeking a skilled Nursing Care Coordinator to join our team in the community health setting. In this role, you will play a crucial part in assessing, planning, and implementing care strategies tailored to individual patients.
As a Field HSS Clinical Coordinator RN, you will utilize company and community-based resources to establish safe and effective case management plans for members. Your collaboration with patients, families, and healthcare providers is essential in developing individualized plans of care.
You will be responsible for identifying and initiating referrals for social service programs, including financial, psychosocial, community, and state supportive services. Additionally, you will manage care plans throughout the continuum of care as a single point of contact, communicating with all stakeholders to ensure quality coordinated care and services are provided expeditiously.

RequirementsTo be successful in this role, you must have a current unrestricted RN license in the state of Texas, at least 2 years of experience working within the community health setting or in a healthcare role, intermediate experience with Microsoft Office, including Word, Excel, and Outlook, reliable transportation, and the ability to travel in assigned regions to visit Medicaid members in their homes and/or other settings.
