Care Coordinator Registered Nurse

About the Role: We are seeking a skilled and compassionate Care Coordinator RN to join our team at Cantex. This is an excellent opportunity for a registered nurse who is passionate about delivering high-quality care and making a meaningful difference in patients' lives.

Responsibilities:

  • Administer nursing care for patients of all ages in their place of residence, coordinating care with the interdisciplinary group, patient, family, and referring agency.
  • Assume responsibility for coordination of care, ensuring seamless transitions and optimal outcomes.
  • Conduct comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs.
  • Develop and implement individualized plans of care, prioritizing patients' needs and goals.
  • Consult with and educate patients and families regarding disease processes, self-care techniques, end-of-life care, nutrition, and dietary needs.
  • Determine scope and frequency of services needed based on acuity and patient/family needs.
  • Initiate preventive and rehabilitative nursing procedures as necessary.
  • Apply concepts of infection control and standard precautions in coordinating and performing patient care activities.

Requirements:

  • Registered Nurse, licensed per state requirements.
  • 2 years of experience with a Hospice agency preferred.
  • Home Care Home Base experience preferred.
  • Good communication skills, assessment skills, and strong clinical documentation skills.
  • Ability to read, write, and speak the English language.
  • Reliable transportation, valid and current auto liability insurance.
