About the Role

We are seeking an experienced Home Health Care Manager to join our team in Houston, TX. The successful candidate will be responsible for supervising and managing all aspects of patient care, ensuring high-quality service delivery and patient progress.

Key Responsibilities
  • Supervise and manage LVN and HHA staff, ensuring effective communication and collaboration among team members.
  • Coordinate visits among disciplines (SN, PT, OT, ST, MSW, HHA) and facilitate interdisciplinary communication to optimize service delivery and patient progress.
  • Initiate referrals to ancillary services and lead weekly case conferences to develop care plans for patients.
  • Establish specific social action plans and realistic goals for each patient, consistent with medically related social and emotional needs.
Requirements
  • Registered Nurse, licensed per state requirements.
  • 2 years of experience in Home Health preferred.
  • Good communication skills, assessment skills, and strong clinical documentation skills.
  • Ability to read, write, and speak English fluently.
