We are currently in search of an MDS Coordinator who possesses extensive experience with PDPM to enhance our dynamic team.

Primary Responsibilities:

  • Hold a valid RN or LPN license in MA, with a minimum of two years of experience in MDS/PDPM.
  • Lead weekly skilled Medicare meetings, ensuring effective communication regarding coverage criteria.
  • Submit MDS documentation to CMS and diligently follow up to address any discrepancies or messages indicated in final validation reports.
  • Conduct PDPM/MDS assessments within required regulatory timelines, including CAAs.
  • Review and assess invoices for skilled patients under consolidated billing as necessary.
  • Maintain an updated PDPM schedule, ensuring all PDPM MDS ARDs are set within the designated time frames in the MDS software.
  • Perform MDS accuracy and PDPM reviews using the required software and processes promptly.
  • Engage in Triple check and Compliance reviews.
  • Monitor skilled documentation for potential improvements or changes.
  • Keep a structured schedule of due dates for all MDS submissions.
  • Coordinate the completion of MDS assessments across all disciplines.
  • Review MDS data for precision and collaborate with relevant staff to ensure accuracy.
  • Finalize and transmit MDS documents in accordance with RAI requirements.
  • Complete Medicare Physician Certification and ensure adherence to CMS Medicare guidelines.
  • Verify timely transmission and approval of assessments by the State.
  • Address any rejected records and prepare them for resubmission.
  • Complete CAAs in line with state regulations.
  • Provide weekly updates regarding managed care residents.

Qualifications:

  • Prior experience in a hospital, long-term care facility, or similar healthcare environment.
  • Strong knowledge of general, rehabilitative, and restorative nursing practices, as well as laws and regulations governing long-term care.
  • Proficient in computer applications, with preferred experience in Accu Med.
  • Demonstrated experience in completing MDS/PDPM assessments is essential.
  • Able to work independently and manage tasks effectively.
  • Flexible availability for month-end processes.
  • Detail-oriented with the ability to complete assessments accurately and promptly.
