MDS Coordinator with PDPM Experience

Sunny Acres Skilled Nursing and Rehabilitation is seeking a highly skilled MDS Coordinator with PDPM experience to join our team. The ideal candidate will have a current RN or LPN license in MA and previous experience in MDS/PDPM.

Key Responsibilities:

* Facilitate weekly skilled Medicare meetings and provide clear communication regarding skilled coverage criteria
* Transmit MDS to CMS and follow up to validate any errors or messages revealed on final validation reports
* Complete PDPM/MDS assessments within regulatory time frames including CAAs
* Participate and review invoices as indicated for skilled patients under consolidated billing
* Maintain a current PDPM schedule while setting all PDPM MDS ARDs within time frames in the MDS software program
* Complete MDS accuracy/PDPM reviews with required software/process in a timely manner
* Participate in Triple check/Compliance reviews
* Monitor skilled documentation for potential opportunities/changes
* Maintain and follow a schedule of due dates for all MDS
* Coordinate the completion of the MDS by all disciplines
* Review the MDS data for accuracy and meet with appropriate staff as needed to assure accuracy
* Finalize and transmit the MDSs per RAI requirements
* Complete Medicare Physician Certification and monitor compliance with CMS Medicare guidelines
* Verify that assessments have been transmitted and approved by the State in a timely manner
* Correct any rejected records and prepare them for re-submission
* Complete CAAs according to state guidelines
* Provide weekly updates for managed care residents

Requirements:

* Previous experience in a hospital, long-term facility, or other healthcare related facility
* Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care
* Computer skills-Windows applications, computer experience Accu Med experience preferred
* Previous experience in completing MDS/PDPM is required
* Can work independently
* Flexible with schedule for month end close
* Detail oriented and able to accurately and timely complete assessments
