Key Responsibilities
  • Facilitate weekly skilled Medicare meetings and provide clear communication regarding skilled coverage criteria.
  • Transmit MDS to CMS and follow up to validate any errors or messages revealed on final validation reports.
  • Complete PDPM/MDS assessments within regulatory time frames, including CAAs.
  • Participate in Triple check/Compliance reviews and monitor skilled documentation for potential opportunities/changes.
  • Maintain a current PDPM schedule and set all PDPM MDS ARDs within time frames in the MDS software program.
  • Complete MDS accuracy/PDPM reviews with required software/process in a timely manner.

  • Coordinate the completion of the MDS by all disciplines and review the MDS data for accuracy.
  • Finalize and transmit the MDSs per RAI requirements and complete Medicare Physician Certification.
  • Verify that assessments have been transmitted and approved by the State in a timely manner and correct any rejected records.
  • Complete CAAs according to state guidelines and provide weekly updates for managed care residents.
Requirements
  • Previous experience in a hospital, long-term facility, or other healthcare-related facility.
  • Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures and laws, regulations, and guidelines governing long-term care.
  • Computer skills, including Windows applications and Accu Med experience preferred.
  • Previous experience in completing MDS/PDPM is required.
  • Ability to work independently and be flexible with schedule for month-end close.
  • Detail-oriented and able to accurately and timely complete assessments.
