Travel Nurse Case Manager Job Description

We are seeking a skilled RN Case Manager to join our team at Travel Nurse Across America. As a RN Case Manager, you will play a critical role in advocating for patients, evaluating their needs, and developing comprehensive care plans.

Key Responsibilities:

  • Evaluate and assess new patients to identify their needs and develop effective care plans.
  • Create and update comprehensive short and long-term patient care plans to ensure optimal outcomes.
  • Educate patients and their families about available resources and healthcare options.
  • Collaborate with healthcare professionals, including RNs, doctors, and social workers, to ensure seamless care coordination.
  • Properly document and submit medical records to ensure accurate and timely communication.
  • Communicate with insurance companies to resolve coverage issues and advocate for patients' needs.
  • Analyze data to identify trends and areas for improvement, and make recommendations to management to optimize patient outcomes and reduce costs.

Requirements:

  • Current RN licensure and 2 years of recent case management experience.
  • Basic Life Support (BLS) certification.
  • Ability to stand and walk for long periods, carry up to 10 pounds, and perform other physical demands of the role.
