Job Title: Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at the Regional Center of the East Bay. As a Case Manager, you will play a vital role in supporting individuals with developmental disabilities and their families in achieving their goals and living fulfilling lives.

Key Responsibilities:
  • Conduct assessments and evaluations to identify individual needs and develop personalized plans
  • Facilitate the planning process and develop Individual Program Plans (IPPs) or Individual Family Service Plans (IFSPs)
  • Monitor and evaluate services provided through the plan, ensuring that individuals receive the necessary support and resources
  • Advocate on behalf of consumers to protect their rights and ensure they receive the services and benefits to which they are entitled
  • Maintain accurate and up-to-date documentation and records
Requirements:
  • Bachelor's degree in Social Work, Psychology, or a related human services field, or equivalent experience
  • 1 year of experience working with individuals with developmental disabilities, or equivalent experience
  • Ability to communicate effectively, both verbally and in writing
  • Ability to work effectively with culturally diverse populations
  • Ability to speak Korean
