Key Responsibilities:
The RN Maternity Care Manager II plays a crucial role in supporting pregnant members who require care coordination services, ensuring optimal health throughout pregnancy, delivery, and the postpartum period. This position is responsible for:
  • Engaging with pregnant members and healthcare providers to assess, plan, and establish personalized health goals.
  • Facilitating and coordinating care for pregnant members while ensuring quality and cost-effective resource utilization.
  • Acting as a single point of contact and advocate for pregnant members enrolled in the Bright Start Maternity program.
  • Conducting comprehensive assessments to identify care coordination needs, including physical, psychosocial, environmental, and support requirements.
  • Creating tailored care plans to help pregnant members overcome challenges and achieve their health objectives.
  • Setting both short-term and long-term goals with associated timelines for completion.
  • Documenting progress towards goals and addressing any issues that arise.
  • Implementing appropriate interventions based on members' needs and clinical progress, scheduling follow-up calls, and making necessary referrals.
  • Coordinating care and services with members, families, OBGYNs, PCPs, specialists, and community-based organizations as needed.

Qualifications:
To be considered for this role, candidates must:
  • Reside in Philadelphia or Montgomery counties in Pennsylvania.
  • Be willing to engage in community events that support members in our maternity program.
  • Hold a current and unrestricted Registered Nurse license in Pennsylvania.
  • Possess a Bachelor's Degree in Nursing.
  • Have a minimum of 3 years of clinical experience in Labor and Delivery, Mother-Baby, or High-Risk Ante-Post Partum care.
  • Have at least 3 years of experience in case management, preferably in a telephonic capacity.
  • Have case management experience within a managed care organization.
  • Hold certification as a case manager (preferred).
  • Demonstrate proficiency in MS Office (Word, Excel, Outlook), internet applications, and electronic medical record systems.
  • Exhibit strong organizational and communication skills, with the ability to meet deliverables within designated timeframes.
  • Possess a valid driver's license and car insurance, along with access to a personal vehicle.
