Nurse Care Coordinator

We are seeking a highly skilled Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will work closely with clients and their families to assess their healthcare needs and develop personalized plans of care.
Key Responsibilities- Assess clients with chronic medical conditions and identify their healthcare needs
- Develop and implement plans of care to optimize healthy outcomes
- Document all client encounters and maintain accurate medical records
- Provide care coordination services and education to clients and their families
- Consult and coordinate care with internal and external healthcare professionals
- Bachelor's degree in nursing from an accredited college or university
- One year of PHN experience and current Georgia license to practice as a Registered Professional Nurse
- CPR certification
- Bilingual English/Spanish
- Experience with case management, children and adolescents with chronic medical conditions and special healthcare needs
