Job Summary

We are seeking a highly organized and detail-oriented Patient Care Coordinator to join our team at Amedisys, Inc. As a key member of our healthcare operations team, you will be responsible for scheduling patient visits, coordinating care, and ensuring the efficient utilization of clinical resources.

Key Responsibilities
  • Schedule patient visits and ensure timely completion of assessments and Oasis visits
  • Prepare weekly clinician schedules and review daily to optimize capacity and productivity
  • Assign and schedule clinicians in the most efficient geography, maximizing clinician efficiency and productivity
  • Manage scheduling issues and refer clinical and performance-related issues to the Clinical Manager
  • Collaborate with field clinicians to ensure all patient visits are scheduled and completed as ordered
  • Monitor hospitalized patients and communicate with the team to ensure follow-up as necessary
  • Complete requested schedules and ensure all orders and services are included as required
  • Support the Clinical Manager and work collaboratively to ensure timely communication with patients, clinicians, and referral sources
Requirements
  • Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse specific to the state of employment
  • High School diploma or GED equivalent
  • One (1+) year of administrative experience in a healthcare environment
  • Preferred: One (1+) year of scheduling experience in a healthcare environment
