Job Summary:

ResCare Community Living is seeking a skilled Nurse Care Coordinator to join our team. As a Nurse Care Coordinator, you will play a vital role in providing high-quality care and support to our clients.

Key Responsibilities:

  • Provide rehabilitation and coordinate medical needs for clients
  • Monitor and administer medications as needed
  • Assist with budget management and ensure that care is delivered within allocated resources
  • Conduct assessments and develop personalized care plans
  • Evaluate the competency and skills of direct care staff
  • Communicate effectively with healthcare professionals and staff regarding client needs and changes in condition
  • Report incidents and accidents, and notify relevant authorities as necessary
  • Support client choice and independence, and promote healthy living
  • Contribute to the development and maintenance of nursing, pharmacy, infection control, and health and safety policies and procedures

Requirements:

  • Bachelor's degree in nursing or a related field from an accredited institution
  • Or, an RN from an accredited two-year program
