Northside Hospital Inc. is seeking a highly skilled Care Coordinator, RN to join our team. As a Care Coordinator, you will play a vital role in ensuring the best possible outcomes for our patients.
Key Responsibilities- Collaborate with patients, families, medical staff, and community agencies to determine the most appropriate setting of care and assess post-hospital care needs.
- Provide interventions to ensure effective and efficient clinical resource management.
- Offer support and guidance to patients and families during hospitalization, including crisis intervention and psychosocial assessment.
- Graduate of an accredited Registered Nursing School (RN) with a current GARN license.
- At least one year of experience in case management, utilization review, or social services.
- Excellent oral and written communication skills.
- Ability to type accurately at an approximate rate of 40-45 wpm.
- BSN degree.
- Three years of experience in case management, social services, or a related field.
- Certification as a Certified Case Manager.
This is a full-time position working 7am-7pm, Monday to Friday, with weekend requirements.
