Job Summary:
Mercy is seeking a skilled RN - ADT Admission, Discharge, and Transfers professional to join our team. As a key member of our healthcare team, you will be responsible for ensuring a smooth patient flow process, from admission to discharge.
Key Responsibilities:
- Conduct thorough admission assessments on patients within 24 hours of admission.
- Identify potential barriers to discharge and develop strategies to overcome them.
- Collaborate with interdisciplinary teams to ensure timely and efficient patient discharge.
- Maintain accurate and up-to-date patient records.
- Ensure compliance with regulatory requirements and hospital policies.
Requirements:
- Graduate of an accredited nursing program (Associate's Degree, Diploma, or BSN).
- Current RN licensure in the state of hire or compact licensure.
- Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire.
