Job Summary

We are seeking a skilled Clinical Care Coordinator to join our team at Springfield Behavioral Hospital LLC. As a key member of our clinical staff, you will play a vital role in ensuring the delivery of high-quality patient care.

Key Responsibilities
  • Conduct thorough intake interviews to assess patient needs and develop personalized care plans.
  • Provide exceptional patient liaison services, ensuring seamless communication between patients, families, and healthcare providers.
  • Collaborate with multidisciplinary teams to coordinate patient care, including referrals, admissions, and discharge planning.
  • Utilize clinical expertise to assess patient psychosocial and emotional status, identifying areas for intervention and support.
  • Develop and maintain effective relationships with patients, families, and healthcare providers to ensure optimal patient outcomes.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure compliance and excellence in patient care.
Requirements
  • Master's degree in a relevant field (e.g., social work, counseling, psychology) or equivalent experience.
  • Minimum 2 years of experience in a clinical or healthcare setting, with a focus on patient care coordination and liaison services.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines.
  • Strong analytical and critical thinking skills, with a focus on patient-centered care.
