Peer Advocate Specialist

The Recovery Advocate Specialist plays a vital role in providing support, advocacy, and linkage services to individuals and their affected families. This position is responsible for engaging people on the street and in hospital emergency rooms, developing brief person-centered service plans, and providing referrals and linkages to needed services.
The ideal candidate will have a valid driver's license and relevant life experience, including personal experience living with a disability, history of substance use, or social/emotional challenge. They will be able to draw upon this experience to serve as a role model and support the recovery goals of individuals served.
The Recovery Advocate Specialist will work collaboratively with Hospital SBIRT Health Coaches to meet with individuals and/or family/significant others at local hospitals ED's or agreed-upon locations. They will assess the needs of the individuals and connect them to treatment and other support services across Nassau and/or Suffolk Counties.
The specialist will remain in an active role and provide follow-up on the individual and/or families at 30, 60-, 90-, 120-, and 180-days' post-discharge from ED. They will collect necessary data to assist in reporting to funding sources and/or hospital systems.
The Recovery Advocate Specialist will also act as a liaison between the Program and the communities it serves. They will represent the Program in a professional and knowledgeable manner and assist in the promotion of the Program through information and education about its services to the communities.
The ideal candidate will have excellent communication and interpersonal skills, with the ability to cultivate a culture of inclusion for all employees. They will be able to identify and implement quality improvement trainings and activities as part of professional development goals.
