Physicians - The patient's Attending Physician oversees care, while Hospice Physicians collaborate with hospice staff and the attending physician.

Nurses (RN or LPN)
Social Workers
Certified Nursing Assistants (Home Health Aides)
Spiritual Care (Chaplains)
Pharmacists
Trained Volunteers - provide companionship for patients and respite for families

Position Overview:


Accountable for the comprehensive direct nursing care of all patients, adhering to established standards of care, hospice, hospital, and unit policies, procedures, and guidelines.


Key Responsibilities:
  • Deliver direct patient care.
  • Evaluate the comprehensive nursing needs of an assigned group of patients (including, but not limited to home health patients).
  • Facilitate the patient and family/significant other education process.
  • Initiate patient start of care through the established admission process.
  • Continuously plan, implement, and assess patient care, involving patients and families/significant others in the process.
  • Collaborate with physician partners and other interdisciplinary team members to develop and execute patient and family/significant other care plans.
  • Effectively communicate all pertinent information (e.g., patient details, safety concerns, equipment issues, quality matters, etc.) to the immediate supervisor in a timely manner.
  • Document and communicate changes in patient condition promptly to team members, DPOA, and physician.
  • Provide nursing care in accordance with established standards of care, hospice, hospital, and unit guidelines, policies, and procedures.
  • Act as a patient advocate.
  • Assist colleagues in completing their responsibilities when time allows.
  • Recognize situations where appropriate assistance is required to effectively carry out the nursing process.
  • Promote positive public relations between patients, family members, the hospital, and the community.
  • Practice within the scope of the Registered Nurse Practice Act.


Qualifications:

Mandatory for the position:
Bachelor's in Nursing (BSN) required within 5 years of employment if applicable
Licensed as a Registered Nurse in the State of Washington
Current Healthcare Provider BLS certification by date of hire
Current Washington State Driver's license and proof of insurability

2024 Benefits Guide

EvergreenHealth is dedicated to delivering the highest quality of care to our patients. Research indicates that possessing a Bachelor of Science in Nursing (BSN) significantly enhances the quality of care provided.

To that end, this position necessitates a BSN or a commitment to initiate a BSN program within three years and obtain a BSN within five years of employment.

Please note that this BSN requirement does not apply to those with an RN license for twenty or more years or current EvergreenHealth RNs hired prior to April 1, 2014.

This position is governed by a collective bargaining agreement between EvergreenHealth and the Washington State Nurses Association (WSNA). Union membership is optional.
