Physicians - The patient's Attending Physician oversees care, while Hospice Physicians collaborate with hospice staff and the attending physician.

Pharmacists
Trained Volunteers - provide companionship for patients and respite for families.

Position Overview:
Responsible for delivering comprehensive nursing care to all patients in accordance with established standards of care, hospice, hospital, and unit policies, procedures, and guidelines.

Key Responsibilities:
  • Deliver direct patient care.
  • Assess the comprehensive nursing needs of an assigned group of patients (including home health patients).
  • Coordinate the education process for patients and families/significant others.
  • Initiate patient start of care through the established admission process.
  • Continuously plan, implement, and evaluate patient care, involving patients and families/significant others in the process.
  • Collaborate with physician partners and interdisciplinary team members to develop and implement care plans.
  • Effectively communicate relevant information to supervisors in a timely manner.
  • Document changes in patient condition promptly to team members, DPOA, and physicians.
  • Provide nursing care in accordance with established standards, policies, and procedures.
  • Act as a patient advocate.
  • Assist colleagues in completing their duties when time allows.
  • Recognize when additional assistance is required to execute the nursing process effectively.
  • Promote positive public relations between patients, families, the hospital, and the community.
  • Practice within the scope of the Registered Nurse Practice Act.

Qualifications:
Required:
  • Bachelor's in Nursing (BSN) required within 5 years of employment if applicable.
  • Licensed as a Registered Nurse in the State of Washington.
  • Current Healthcare Provider BLS certification by date of hire.
  • Current Washington State Driver's license and proof of insurability.

This position requires a BSN or a commitment to initiate a BSN program within three years and complete it within five years of employment.

Please note this BSN requirement does not apply to those with an RN license for twenty or more years or current EvergreenHealth RNs hired prior to April 1, 2014.

This position is governed by a collective bargaining agreement between EvergreenHealth and the Washington State Nurses Association (WSNA). Union membership is optional.
