Job Summary

V-Tech Solutions, Inc. is seeking a detail-oriented and passionate Licensed Graduate Social Worker (LGSW) to support our client in the D.C. area. As a Care Manager, you will provide comprehensive care coordination services to members enrolled in the Health Services for Children with Special Needs (HSCSN) health plan.

Key Responsibilities
  • Manage a caseload of members with moderate to complex medical and psychosocial needs.
  • Assess members' needs, develop, monitor, and evaluate treatment plans.
  • Coordinate care and refer members to appropriate services.
  • Communicate with members, caregivers, and healthcare providers to ensure seamless care coordination.
  • Participate in multidisciplinary meetings to discuss members' cases and develop care plans.
  • Document members' interactions and care coordination activities according to organizational policy.
  • Collaborate with Senior Care Managers to review and prioritize cases, set objectives, and identify potential risks and utilization concerns.
Requirements
  • Master's Degree in Social Work or related field.
  • Two or more years of experience in healthcare or related field, with a focus on care management and coordination.
  • Licensed in DC as a Licensed Graduate Social Worker (LGSW).
  • Certified Case Manager (CCM) certification preferred.
Preferred Qualifications
  • Bilingual (Spanish speaking) preferred.
  • Proficiency in Microsoft Office suite and computer systems.
  • Strong organizational and time management skills.
  • Ability to work under pressure and meet deadlines while maintaining a positive attitude and providing excellent customer service.
