Position Overview:
The Healthcare Relationship Manager is tasked with fostering and sustaining positive connections with clients to enhance referrals for various services including Home Health, Hospice, Home Infusion, PACE, and Transitional Care Unit within designated accounts.

Essential Qualifications:
- Education aligned with the discipline and relevant licensure as required.
- Bachelor's Degree in Social Work.
- Current state professional license or certification relevant to the discipline upon hire.
- Minimum of 3 years of experience in a clinical setting.

Preferred Qualifications:
- Master’s Degree in Social Work.
- Bachelor's Degree in a relevant clinical discipline (e.g., Respiratory Therapy).
- Experience in liaison, marketing, or sales roles.
- Background in the healthcare industry.
- Proven track record in successful medical account management.
- Familiarity with Epic EMR.
