Wound Care Registered Nurse

POSITION OBJECTIVE
The role of a Registered Nurse in Home Health Care involves the continuous evaluation, care planning, documentation of patient progress, and accountability for assigned patients within the home health care framework. The RN will manage on-call duties as scheduled with a commitment to responsibility.
KEY RESPONSIBILITIES
1. Patient Care Management: Employs the nursing process to assess, plan, implement, and evaluate care tailored to the unique physical and emotional needs of each patient, particularly those with wound or skin conditions.
2. Staff Support: Provides guidance and direct support to nursing personnel, ensuring they maintain up-to-date knowledge in the management of patients with compromised skin integrity, complex wounds, and fistulas.
3. Interdisciplinary Collaboration: Works in partnership with physicians, case managers, and other healthcare professionals to address and fulfill the needs of patients with altered skin integrity and complex wound care requirements.
4. Age-Specific Care: Demonstrates the necessary knowledge and skills to deliver care that is appropriate for the disease/diagnosis and age of the patients served by Bridge Personal Care Services.
5. Communication and Coordination: Establishes and maintains effective communication and collaboration with the Interdisciplinary Team and the primary physician regarding patient conditions, care plans, and anticipated needs to ensure outstanding patient care. Utilizes a team-oriented approach for educational opportunities and family care matters.
6. Caseload Management: Maintains an assigned patient caseload within scheduled hours while minimizing overtime.
7. Admission Criteria Communication: Effectively communicates the criteria for admission, resumption, and recertification specific to each patient.
8. Documentation Standards: Ensures documentation meets or exceeds the guidelines set by Bridge Personal Care Services.
9. On-Call Management: Responsible for managing on-call duties and ensuring that management is informed of any unusual or significant incidents.
10. Budget-Conscious Care: Consistently maintains high-quality patient care while adhering to budgetary constraints.
11. Service Excellence: Exemplifies Service Excellence by embodying the mission, vision, and values of the organization to enhance patient satisfaction.
12. Compliance: Adheres to organizational policies, procedures, and practices to ensure quality job performance and compliance with relevant regulations.
REQUIRED QUALIFICATIONS
- Valid R.N. licensure for the state of Ohio.
- A minimum of 1-2 years of general nursing experience.
- Commitment to the agency's philosophy, mission, and goals, along with knowledge of applicable standards and regulations.
- A valid driver's license is required, and compliance with company fleet policy is necessary.
- Strong service-oriented interpersonal and communication skills.
- Ability to demonstrate the knowledge and skills necessary to provide age-appropriate care.
- This position is classified as "at risk" for potential exposure to bloodborne pathogens.
PREFERRED QUALIFICATIONS
- Experience in homecare settings is preferred.
PHYSICAL REQUIREMENTS
This position necessitates a full range of physical movements, including walking, lifting, bending, squatting, kneeling, twisting, and standing. The individual must be capable of walking for up to one hour daily, sitting for four to six hours, and standing for one hour. The ability to lift between twenty to fifty pounds and reach above shoulder height is essential. Good eye-hand coordination and fine motor skills are required for tasks involving simple grasping. Excellent verbal communication skills are necessary for daily operations. The individual must have corrected vision and hearing within normal ranges and be able to operate a vehicle for business travel.
