Wound Care Registered Nurse

POSITION OBJECTIVE
The Registered Nurse in Home Health Care is responsible for continuous evaluation, care planning, documentation of patient progress, and accountability for assigned patients within the home health care framework. This role includes managing on-call duties as scheduled with a high level of responsibility.
KEY RESPONSIBILITIES
1. Patient Care Management: Employs the nursing process to assess, plan, implement, and evaluate care tailored to meet the unique physical and emotional needs of each patient, particularly those with wound or skin care requirements.
2. Staff Support: Provides guidance and direct support to nursing personnel, ensuring they maintain up-to-date knowledge in the management of patients with compromised skin integrity, complex wounds, and fistulas.
3. Interdisciplinary Collaboration: Works closely with physicians, case managers, and other healthcare professionals to address and fulfill the needs of patients, especially those with complex wound care requirements.
4. Age-Specific Care: Demonstrates the necessary knowledge and skills to deliver care that is appropriate for the age and diagnosis of the patients served by Bridge Personal Care Services.
5. Communication and Coordination: Establishes and maintains effective communication and collaboration with the Interdisciplinary Team and the patient's primary physician regarding the patient's condition, treatment orders, care plan, and anticipated needs to ensure high-quality patient care. Engages in a team approach to patient education and family involvement.
6. Caseload Management: Effectively manages an assigned patient caseload within scheduled hours while minimizing the need for overtime.
7. Admission and Recertification Communication: Clearly communicates the criteria for patient admission, resumption, and recertification.
8. Documentation Standards: Ensures that documentation meets or exceeds the guidelines set by Bridge Personal Care Services.
9. On-Call Management: Responsible for managing on-call duties and assisting with helper assignments, including keeping management informed of any unusual incidents.
10. Budget-Conscious Care: Consistently provides optimal patient care while adhering to budgetary constraints.
11. Service Excellence: Exhibits Service Excellence by embodying the mission, vision, and values of the organization to enhance patient satisfaction.
12. Compliance: Adheres to organizational policies, procedures, and practices to ensure quality job performance and compliance with Medicare/Medicaid regulations, including OASIS when applicable.
ESSENTIAL QUALIFICATIONS
- Valid R.N. licensure for the state of Ohio.
- A minimum of 1-2 years of general nursing experience.
- Understanding of the agency's philosophy, mission, and regulatory standards.
- A valid driver's license is required, and compliance with company fleet policy is necessary.
- Strong interpersonal and communication skills.
- Ability to demonstrate knowledge and skills necessary for age-appropriate patient care.
- This position is classified as 'at risk' for potential exposure to bloodborne pathogens.
PREFERRED QUALIFICATIONS
- Experience in homecare settings is preferred.
PHYSICAL REQUIREMENTS
This role necessitates a full range of physical movements, including walking, lifting, bending, squatting, kneeling, twisting, and standing. The individual must be capable of walking for up to one hour daily, sitting for four to six hours, and standing for one hour. The ability to lift between twenty to fifty pounds and reach above shoulder height is essential. Good eye-hand coordination and fine motor skills are required for various tasks. Excellent verbal communication skills are necessary for daily responsibilities. The individual must have corrected vision and hearing within normal ranges and be able to operate a motor vehicle for work-related travel.
