POSITION OBJECTIVE
The role of a Registered Nurse in Home Health Care is to oversee continuous evaluation, care planning, documentation of patient progress, and accountability for assigned patients within the home health care framework. Responsible for managing on-call duties as scheduled with reliability.
KEY RESPONSIBILITIES
1. Patient Care Management: Employs the nursing process to assess, plan, implement, and evaluate care tailored to meet the unique physical and emotional needs of each patient, particularly those with wound or skin conditions.
2. Staff Support: Provides guidance and direct support to nursing personnel, ensuring they maintain up-to-date knowledge in the management of patients with compromised skin integrity, complex wounds, and fistulas.
3. Interdisciplinary Collaboration: Works in conjunction with physicians, case managers, and other healthcare professionals to address and fulfill the needs of patients with altered skin integrity and complex wound issues.
4. Age-Specific Care: Demonstrates the necessary knowledge and skills to deliver care appropriate to the disease/diagnosis and age of the patients served by Bridge Personal Care Services.
5. Communication and Coordination: Establishes and maintains effective communication and collaboration with the Interdisciplinary Team and the primary physician regarding patient status, care plans, and anticipated needs to ensure high-quality patient care. Utilizes a team-oriented approach for educational opportunities and family care matters.
6. Caseload Management: Maintains an assigned patient caseload within scheduled hours while effectively minimizing overtime.
7. Admission Criteria Communication: Capable of articulating admission, resumption, and recertification criteria specific to each patient.
8. Documentation Standards: Ensures documentation aligns with or surpasses the guidelines set by Bridge Personal Care Services.
9. On-Call Management: Oversees on-call responsibilities and helper assignments, ensuring management is informed of any unusual or special incidents.
10. Budget Management: Consistently upholds optimal patient care while adhering to budgetary constraints.
11. Service Excellence: Exhibits Service Excellence by embodying the mission, vision, and values of the organization to enhance patient satisfaction.
12. Compliance: Adheres to organizational policies, procedures, and practices to ensure quality job performance and compliance with Medicare/Medicaid regulations, including OASIS when applicable.
ESSENTIAL QUALIFICATIONS
- Possession of a valid R.N. license for the state of Ohio.
- A minimum of 1-2 years of general nursing experience.
- Commitment to and understanding of the agency's philosophy, mission, and regulatory standards.
- A valid driver's license is required, and compliance with company fleet policy is necessary.
- Strong service-oriented interpersonal and communication skills.
- Ability to demonstrate the knowledge and skills necessary to provide age-appropriate care for patients served.
- This position is classified as "at risk" for potential exposure to bloodborne pathogens.
PREFERRED QUALIFICATIONS
- Experience in homecare settings is preferred.
PHYSICAL REQUIREMENTS
This position necessitates a full range of physical motions, including intermittent walking, lifting, bending, squatting, kneeling, twisting, and standing. The individual must be capable of walking for up to one hour daily, sitting for four to six hours, and standing for one hour. Ability to lift between twenty to fifty pounds and reach overhead is required. Good eye-hand coordination and fine motor skills for simple grasping tasks are essential. Excellent verbal communication skills are necessary for daily responsibilities. The individual must have corrected vision and hearing within normal ranges and be able to operate a motor vehicle for business-related travel.
