Wound Care Registered Nurse

POSITION OBJECTIVE
The role of a Registered Nurse in Home Health Care is to take responsibility for ongoing evaluation, care planning, documentation of patient progress, and accountability for assigned patients within the home health care framework. This includes managing on-call duties as scheduled in a reliable manner.
KEY RESPONSIBILITIES
1. Patient Care Management: Employs the nursing process to assess, plan, implement, and evaluate care tailored to meet the unique physical and emotional needs of each patient, particularly those with wound or skin care requirements.
2. Staff Support: Provides guidance and direct support to nursing personnel, ensuring they remain knowledgeable about managing patients with compromised skin integrity, complex wounds, and fistulas.
3. Interdisciplinary Collaboration: Works closely with physicians, case managers, and other healthcare professionals to address and fulfill the needs of patients with altered skin integrity and complex wound issues.
4. Age-Specific Care: Demonstrates the necessary knowledge and skills to deliver care that is appropriate for the age and specific needs of the patients served by Bridge Personal Care Services.
5. Communication and Coordination: Establishes and maintains effective communication and collaboration with the Interdisciplinary Team and the primary physician regarding patient conditions, care plans, and anticipated needs to ensure high-quality patient care. Utilizes a team approach to address teaching opportunities and family care issues.
6. Caseload Management: Maintains an assigned patient caseload within scheduled hours while minimizing overtime.
7. Admission Criteria Communication: Effectively communicates admission, resumption, and recertification criteria specific to each patient.
8. Documentation Standards: Ensures that documentation meets or exceeds the guidelines set by Bridge Personal Care Services.
9. On-Call Management: Manages on-call responsibilities and keeps management informed of any unusual or special incidents.
10. Budget-Conscious Care: Consistently maintains optimal patient care while adhering to budget constraints.
11. Service Excellence: Exhibits Service Excellence by embodying the mission, vision, and values of the organization to enhance patient satisfaction.
12. Compliance: Adheres to organizational policies, procedures, and practices to ensure quality job performance and compliance with Medicare/Medicaid regulations.
REQUIRED QUALIFICATIONS
- Valid R.N. licensure for the state of Ohio.
- Minimum of 1-2 years of general nursing experience.
- Understanding of the agency's philosophy, mission, and regulatory standards.
- Possession of a valid driver's license (must obtain an Ohio driver's license within one year of residency if not already held) and compliance with company fleet policy.
- Strong interpersonal and communication skills with a service-oriented approach.
- Ability to demonstrate knowledge and skills necessary to provide age-appropriate care for patients.
- This position is classified as "at risk" for potential exposure to bloodborne pathogens.
PREFERRED QUALIFICATIONS
- Experience in homecare settings is preferred.
PHYSICAL REQUIREMENTS
This position necessitates a full range of body movements, including walking, lifting, bending, squatting, kneeling, twisting, and standing. The individual must be capable of walking for up to one hour daily, sitting for four to six hours, and standing for one hour. The ability to lift between twenty to fifty pounds and reach above shoulder height is required. Good eye-hand coordination and fine motor skills for grasping tasks are essential. Excellent verbal communication skills are necessary for daily operations. The individual must have corrected vision and hearing within normal ranges and be able to operate a motor vehicle for business-related travel.
