Job Summary:

EvergreenHealth seeks a skilled Social Worker to join our team in a Cancer Registry Support role. As a key member of our Oncology program, you will work in partnership with leadership and healthcare professionals to ensure continuity and coordination of care for adult patients and families.

Key Responsibilities:
  • Assume a daily case load, identifying and coordinating cancer support resources, including legal, financial, and social support, living conditions, and available caregivers.
  • Provide consultation to oncology clinical staff on the psychosocial aspects of patient care and available services and resources.
  • Verbally and visually assess patients through interviews to gather necessary data about their situation, stresses, functional status, financial systems, and support systems.
  • Perform other duties as assigned.
Requirements:
  • Masters degree in Social Work.
  • One of the following credentials is required: Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW).
  • 2 years of Inpatient, Outpatient, and/or Mental Health social work experience.
  • Oncology setting preferred or community setting experience.
  • Age-specific or behavior-specific knowledge and experience in working with specific patient populations.
  • Oral and written language skills, visual and aural skills to effectively communicate and provide patient care.
  • Accountability for professional practice, commitment to patient care, and commitment to uphold the Patient Bill of Rights.
  • Current CPR Healthcare Provider certification.
  • Excellent organizational skills and ability to work well under stress.
  • Excellent customer service and communication skills.
  • Self-directed and able to work independently and as a team member.
Desired Qualifications:
  • 3 years of experience working in oncology, hospice, or grief services.
