Job Summary

We are seeking a skilled and compassionate Home Health Skilled Nursing Visits RN/LPN to join our team in Colorado. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.

Key Responsibilities
  • Provide skilled nursing care to patients in their homes, assessing their needs and developing personalized care plans.
  • Work independently to implement interventions and maintain accurate documentation of patient assessments and outcomes.
  • Communicate effectively with patients, families, and healthcare professionals to ensure coordinated care.
  • Adhere to all regulatory requirements and company policies and procedures.
Qualifications
  • Valid RN or LPN license in the state of Colorado.
  • Minimum of 1-2 years of nursing experience, preferably in home health or a similar setting.
  • Excellent clinical assessment and critical thinking skills.
  • Proficiency in computer usage and documentation.
  • Strong communication and interpersonal skills.
