Home Health LPN

At CenterWell Home Health, we're dedicated to providing personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. As a Home Health LPN, you'll play a vital role in our care teams, working alongside nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers to help patients rehabilitate, recover, and regain their independence.
Key Responsibilities:- Provide professional nursing services under the direction of a RN, adhering to state Nurse Practice Act, licensure/certification requirements, healthcare standards, governmental laws, and CenterWell Home Health policies and procedures.
- Deliver skilled nursing interventions to address patients' rehabilitative needs, preventative care, and treatment of their illness.
- Utilize a holistic approach to provide problem-specific therapeutic interventions, teaching, and training activities in accordance with the plan of care.
- Collaborate with the RN Supervisor to identify needed changes in the Plan of Treatment and communicate verbal orders from physicians, as permitted by state law/regulations/Nurse Practice Act.
- Implement a systematic, individualized goal/outcome-driven approach to nursing care, ensuring patients' needs are met and goals are achieved.
- Maintain open communication with patients, physicians, clinical managers, and other healthcare team members regarding patient/family needs and status changes.
- Participate in care coordination activities and discharge planning as necessary.
- Maintain confidentiality and adhere to professional standards in sharing patient information.
- Ensure timely and effective implementation of the plan of care by communicating with the healthcare team and arranging for necessary supplies, materials, and equipment.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
